Request specific records maintained within a City bureau.

  1. Fill out request form

    Fill out the City's Uniform Public Records request form.

    A fee may apply to your request, fee schedule can be found here.

  2. Submit completed form to appropriate bureau/office

    A contact list for all bureaus is available here .

    A copy of this form must be submitted to each bureau you want records from.

  3. Bureau/office contact

    You will be notified if additional information is needed to fulfill your request.
